Fees and Insurance
Physician referral is not required. Clients should contact Dr. Zukiwski directely to book appointment.
Initial Consult: The first appointment will often be an initial consult which involves a review of client history and current symptoms and recommendations of treatment options provided by Dr. Zukiwski.
Fees:
Initial consult is $200.
Treatment (one hour) $200
Brain map typically $1800

2021 fees are based on the recommended fee schedule from the Psychologist's Association of Alberta.
Payment: Cash, cheque, etransfer.
Missed Appointments: Dr. Zukiwski reserves the option to charge whole or partial session fee for appointments missed without 24 hour notice.
Insurance: Most third party insurance plans have coverage for services provided by a psychologist ranging between $500 to $1200 per year. Alberta Health Insurance does not cover the assessment or treatment.
Veterans and Military: Dr. Zukiwski is an approved provider for veterans and active service members of the Canadian Forces.
Ages: Children, teens, adults. Minimum age is 10 years.
